PARENT CONSENT
I confirm my child is physically fit and healthy and will undertake to advise you of any change. I consider him/her capable of taking part in gymnastics. I have completed the section on medical details and give consent that in the event of any illness/accident any necessary treatment can be administered. I confirm that I have read through the membership requirements with my child and they understand and agree to abide by the rules.
By agreeing to this statement, I declare that I am aware of the element of risk involved and while I accept that the coaches will take precautions to prevent accidents, I understand that they may not be held responsible for loss, damage or injury to my child. I confirm that my child is a current member of British Gymnastics.

IF YOU HAVE OPTED FOR PHOTOGRAPHIC CONSENT
Lightning School of Gymnastics is keen to promote the gymnastics at our club and like to take photographs and video footage of achievements and the participation of gymnasts at the club and competing. All imagery will be taken in accordance with British Gymnastics Child Protection Policy 2004.
I have discussed this matter with my child and they understand and are in agreement.
The images can be used for Promotional materials, publications, presentations and our Official Website.
I understand that these photographs will be published in line with the guidance contained in the BG Child Protection Policy 2004 on Use of Photographs and Recorded images and the Health Safety and Welfare Policy 2005 on Use of Imagery on the Internet.
I understand that no personal information will be displayed with the image.​

SESSIONS
Session’s start with a warm up to make sure the gymnasts are ready for action! Therefore, please make sure that your child is on time and ready to start the class. If your child is more than 10 minutes late then your child will not be allowed to participate. When the class is finished please make sure that your child does not leave the building until you have arrived to collect them unless otherwise stated in our emergency contact form.
Please ensure that your child has a bottle of water with them on arrival. We will keep all drinks in the gym with us. (Please note that fizzy drinks are not permitted)

TASTER SESSIONS
We ONLY offer £15 trial sessions during set times of the year for our school aged children. Pre-school children can book a £30 three week taster at any time within our term classes. Both can be booked via our website.

EMERGENCY DETAILS KEPT ON FILE.
I confirm that I am happy for my details to be kept on file and for Lightning School Of Gymnastics Limited to access these details in case of an emergency whilst my child attends a gymnastics class. I also confirm that I am happy for these details to be used for registers, organising my child into the correct group, and that I am happy for emails to be sent to me from Lightning School Of Gymnastics Limited concerning events, general queries, invoices and payment reminders that relate to my Childs gymnastics.
PAYMENT
Session fees need to be paid in advance of the next term in full to reserve your place for the following term. An email reminder and booking instructions will be sent out at the middle of each term to our current members with a one week password protected, priority booking period. After the one week, our classes will then be open to the public and waiting list to book. Failure to book within the one week priority booking period may mean losing your child’s place at the club.
Included in these fees will be the cost of assessment Lightning medals, Competition fees, Aerial Rosette certificates, Costume hire if relevant to your child.

If you wish to cancel your child’s membership, we ask for a terms notice. We do not offer refunds. Fees cannot be transferred to other members or used for different terms. Missed classes due to parties, illness or other, can not be replaced. Due to maximum capacity restrictions, if you miss your class you cannot attend a different class that week.
*As from 29/11/22 Bookings cancelled any time up to 7 days before the start of the course commencement date, will be subject to a £10 administration charge fee due to our website provider charging us for any refunds made.

UNFORSEEN CIRCUMSTANCES
In the case of unforeseen circumstances that our classes cannot take place in the gym, classes will not be refunded. We will however do our best to adapt to a situation and provide online classes via zoom.

INSURANCE
All participants that take part in our classes are insured by Insure4sport. All Squad gymnasts must be insured with British Gymnastics.
DRESS CODE
It is important that participants are safe in the gym and therefore we require all gymnasts to wear a leotard. Gymnasts should be bare foot and wear their hair up and off the face. We have a compulsory club uniform leotard priced at £35 that you can purchase through the club at The GAP Centre shop. This must me purchased after three weeks of attendance.
Girls may wear shorts. Boys must wear a leotard/club T shirt and shorts. All jewellry must be removed. For GAP and aerial classes, legging are recommended.

SECURITY
Our centre have CCTV inside of the gym for the safety of our coaches, gymnasts and parents. Footage is only kept for two weeks before being deleted, unless needed to review an accident in the gym or a break in.
